Policy

  • If your insurance requires a referral, it must be in place at the time of your appointment in order for you to be seen.  Please contact your primary care physician for a referral.

  • Co-pays are due at the time of your appointment.  There will be an additional fee for billing of your co-pay.

  • We do charge a $25.00 fee for an appointment missed without notice of cancellation.

  • We accept most major insurances.

  • We accept Visa, Mastercard, Discover, cash or check for payment.  We do not currently accept American Express.
